AGREEMENT FOR ASSUMPTION OF RISK, HOLD HARMLESS, INDEMNIFICATION, AND GENERAL RELEASE

BY REGISTERING FOR THE NCUR 2023 CONFERENCE, YOU ARE WAIVING CERTAIN LEGAL RIGHTS. ALL CONFERENCE PARTICIPANTS MUST READ THIS RELEASE DOCUMENT AND WILL HAVE TWO OPPORTUNITIES TO ACKNOWLEDGE THEIR UNDERSTANDING AND ACCEPTANCE OF THIS RELEASE:  1) ABSTRACT SUBMISSION 2) REGISTRATION PAYMENT.

I, desire to voluntarily participate in and/or attend the 2023 National Conference on Undergraduate Research (“Conference”) hosted by the Board of Regents of the University of Wisconsin System d/b/a the University of Wisconsin – Eau Claire (“University”) and operated by the Council on Undergraduate Research (“CUR”) (together, "Releasees").

I UNDERSTAND THAT I AM BEING ASKED TO READ EACH OF THE FOLLOWING PARAGRAPHS CAREFULLY. I UNDERSTAND THAT IF I WISH TO DISCUSS ANY OF THE TERMS CONTAINED IN THIS AGREEMENT, I MAY CONTACT THE DEPARTMENT OF RISK MANAGEMENT AT SAFETY@UWEC.EDU.

Photo Release Agreement

I grant permission for photos to be taken during the event to be used by the University of Wisconsin-Eau Claire, or agencies for educational/public relations purposes and the Council on Undergraduate Research (CUR) for event marketing purposes.

Assumption of Risks

For and in consideration of the Releasees permitting me to participate in and/or attend the Conference held at UW-Eau Claire, including but not limited to any and all activities, lectures, and sessions from April 12, 2023 to April 15, 2023, hereinafter referred to as the “Conference Activities”, I hereby expressly assume all of the risks associated with the Conference Activities. I acknowledge that I have been advised to have health insurance in effect. I know, understand, and appreciate the risks that are inherent in the Conference Activities. I hereby assert that my participation in the Conference Activities is voluntary and that I knowingly and expressly assume all such risks. I have read and executed this document with full knowledge of its legal significance.

Hold Harmless, Indemnity and Release

In consideration of my participation in the Conference Activities I, for myself, spouse, heirs, personal representatives, estate or assigns, agree to defend, hold harmless, indemnify and release the Releasees and their officers, employees, agents, and volunteers from and against any and all claims, demands, actions, or causes of action of any sort on account of damage to personal property, personal injury, or death which may result from my participation in and/or attendance at the Conference Activities. This release includes claims based on the negligence of the Releasees, and their officers, employees, agents, and volunteers, but expressly does not include claims based on their intentional misconduct or recklessness. I understand that by agreeing to this clause I am releasing claims and giving up substantial rights, including my right to sue, and I hereby waive the right that I have to bargain for a different waiver of liability terms. I have read and executed this document with full knowledge of its legal significance.
